A Letter to Our Community

As the only non-profit home care and hospice agency in Carroll County, VNHCH continually reviews our service offerings to determine the best use of our resources in service of the greater good.
VNHCH recently made the decision to not renew our contract with the State of NH to provide long-term care services supported through New Hampshire State Title XX and IIIB grant programs after June 30, 2026. The programs only covered non-skilled homemaker, home health aide, and personal care services, not skilled nursing home care or hospice services.
These changes in services impacted approximately nine patients. Each patient was notified of the changes and alternative service options ahead of time. Jennifer Robinson, our community health worker, has met with each individual and is actively working to provide resources and support transition planning.
We notified all local healthcare providers in writing of this change in March. We also worked closely with the State on the transition.
VNHCH continues to offer our core services including:
- Skilled nursing care in the home
- Rehabilitative services including physical, occupational and speech therapy.
- Hospice care in the home or facility setting
- Advance care directives preparation
In almost all cases, skilled services and hospice care provided by VNHCH are reimbursed by your insurance carrier, Medicaid or Medicare.
We remain committed to providing these services to enable our community members to remain in their homes.
