About the Role
As an RN Case Manager, you’ll provide compassionate, high‑quality nursing care to patients in the comfort of their homes. You’ll guide patients and families through recovery, chronic illness management, or end‑of‑life care, ensuring dignity, comfort, and exceptional clinical outcomes.

What You’ll Do
- Lead patient care through comprehensive assessments, ongoing evaluations, and individualized care planning
- Deliver skilled nursing, including wound care, symptom management, medication administration, and patient education
- Coordinate services with physicians, therapists, social workers, and home health aides
- Support families with education, emotional guidance, and resources
- Maintain accurate documentation in the EMR to ensure compliance and continuity of care
- Participate in on‑call rotation to support urgent patient needs
- Promote quality outcomes and reduce avoidable hospitalizations
Qualifications
- Active Registered Nurse license in New Hampshire & Maine
- Minimum 1–2 years of clinical nursing experience (home care or hospice preferred)
- Strong assessment and critical‑thinking skills
- Reliable transportation and a valid driver’s license
- Comfort with mobile documentation and EMR systems
